Before being seen by a doctor or dentist, each patient is initially required to call 386-738-6990 to receive a qualifying packet. Alternatively, a patient may pick up the qualifying packet in person, or may download the qualification form from this web site (accessible from the "Who Qualifies?" page). A patient must qualify annually, but otherwise need re-qualify only if there are changes in the patient circumstances. Once having qualified, the patient will be given an appointment to see a health care provider.

The medical clinic will be open for patients by appointment only on Monday, Wednesday, and Friday of each week from 5 p.m. to 9 p.m. The dental clinic will be open for patients by appointment only on Monday and Wednesday of each week from 6 p.m. to 9 p.m. The dental clinic is also open from 9 a.m. to noon on Friday.

Please note that we are not a walk-in clinic, and all patients must be first qualified and then will be given appointments.
