| Our medical clinic is located at 312 West New York Avenue, just west of Clara, and just to the east and across
the street from St. Peter's Catholic Church. The nearest Votran bus stop (either northbound or southbound) is at the north
entrance of the Thomas C. Kelly Administration Center on Rich Avenue. To reach the clinic from either bus stop, proceed west
on Rich Avenue, turn left on Florida Avenue, cross New York Avenue (SR44), then proceed west on New York to the clinic. The
walking distance is approximately 650 yards. Parking is available in the rear of the building for those bringing their own

| Our dental clinic is located at 1245 South Volusia Avenue in Orange City, diagonally across the intersection
from the Blue Springs Car Wash. Parking is available in the front of the building for those bringing their own cars. The
northbound Votran bus stop is nearly across the street from the clinic. The southbound Votran bus stop is about one short block
